If you have questions about this process, please contact your department chair.

Step 2 – Background Checks

La Salle University requires every employee to complete a background screening.

Please sign and return the Authorization and Release for Background Checks so that we can initiate your background screenings. Please note the release form is two pages. Please complete the second page and return both pages.

The following clearances are required of all employees:

  1. Pennsylvania State Criminal Background Check

  3. FBI Fingerprint Check

Click here for instructions on how to obtain and submit clearances.

If you have completed any of these clearances within the past 12 months, then you may submit copies of the documents for review at this time. With your completed clearances, please submit the Disclosure Statement for Certifying Existing Clearances.

Tips for completing the I-9 Form:

Federal regulations require La Salle University to verify your identity and employment eligibility by completing an I-9 form.

Completing the Form

Please fully complete page 1 of the I-9 (Page 2 is for us to complete)

  1. The formatting of the dates must be MM/DD/YYYY
  2. If you make a mistake, please cross out the mistake with one straight line and initial next to it.
  3. Remember to complete the grey box indicating if you used a preparer/translator (see image below as a reference).

Original Forms of ID

In addition to completing page 1 of the I-9, you must provide clear images (front and back) of the original forms of identification you are using to satisfy the requirements of your I-9.  You will need to bring the proper identification with you on or before your first day of work to complete the I-9.  Below are the most commonly presented forms of ID:

  • Original, unexpired U.S. Passport OR
  • Original Social Security Card PLUS a photo ID such as an unexpired driver’s license or student ID card OR
  • Original or certified copy of a state Birth Certificate (note: a “Birth Registration Notice” or a certificate issued by the hospital is not an official birth certificate) PLUS a photo ID such as an unexpired driver’s license or student ID card

The documents above are not the only types of identification that may fulfill the requirement; please see the complete list of acceptable documents in the New Hire Packet for additional information. We can accept one document from list A to satisfy government requirements, or a document from List B accompanied with a document from List C to satisfy government requirements. Foreign students or other non-citizen employees may have different requirements, so please review the form, and contact our office with any questions. Please keep in mind that copied or faxed documents are NOT accepted. Complete instructions for the I-9 form are available in the New Hire Packet.
