Health History Form
Students should complete the health history form, immunization form and upload documentation of immunizations on the Student Health Portal. See the list of required and recommended immunizations below.
Students can reach out to a primary healthcare provider or a former school to obtain accurate current immunization records.
Students do not need a yearly, or even one-time, physical to attend La Salle University. Some majors will require specific proofs of vaccinations or medical certifications and some athletic activities will require a physical and medical testing, but students with these needs will be informed by their program administrators.
PLEASE NOTE: Failing to complete these requirements may eventually result in a medical hold that will restrict students from registering for classes.

HPV (Human Papillomavirus) | 2 dose series if 1st dose received before 15th birthday. • Dose 2 given 6-12 months after 1st. 3 dose series if 1st dose received on/after 15th birthday. • Dose 2 given 1-2 months after 1st. • Dose 3 given 6 months after 1st. |
Meningococcal B | Bexsero: 2 dose series administered at least 30 days apart. Trumenba: 2 or 3 dose series dependent on assessed risk. |
